How do I make Custom Duke Nukem 3D Levels?

I can't find a strait up tutorial on how to do this. I herd there's a book that comes with the game that says how to, but i got my version online so I don't have one. I have herd of programs like 'Mapster',but i don't know how to get those programs. What program do I use and how do I use it?
